Coaches & Parents:

Before any student can engage in any athletic event (including tryouts and clinics) students must have these forms completed in full.  The physicals are good for only one (1) calendar year. All forms must be turned in to the Athletic Director – not the individual coaches. We will not accept any forms unless all forms needed are complete and properly signed. 

Athletic fees: Students must pay or make arrangements to pay their coach prior to the first game of the season. Once final cuts have been made for teams, the Athletic Director will give instructions as to who will be collecting the fees from team members on a specific day.
